Access to Patient Information

The practice is registered with the Data Protection Agency.

All members of the practice team, both clinical and administrative are bound by the practice Confidentiality Policy.
It is a serious disciplinary offence to breach this policy, which could result in dismissal.

Access to your records by all members of the team – including attached staff and administrative staff – is on a ‘need to know’ basis.
If you feel strongly that sensitive information should not be shared with the practice team please discuss this during your consultation with doctor.

Access to your records by outside agencies

Other clinicians – e.g. hospital consultants – access is with your consent for the purpose of referral etc.
Insurance companies, banks, employers – access only with your written consent.
As a training practice we are routinely inspected for the quality of care and training we provide, the persons understanding this work are bound by the same stringent Confidentiality Policy as any other member of the team.

Summary Care Record

You can choose to opt-out and withhold your clinical information from your Summary Care Record on the NHS spine. If you wish to do so please ask reception for the appropriate form to complete.

Access to Records

All patients have the right to access to their medical records (subject to certain limitations).
We are committed to ensuring your records remain confidential, therefore, no information will be discussed without a patients written authority unless we are legally obliged to do so.
